JOB SUMMARY
The Care Coordinator assists the Superintendent with the day-to-day running of the Home. She/ He is expected to serve as a good role model and should be observant, and attentive to the needs of the residents under our care, adopting a holistic care approach by looking into the physical, educational, social, emotional and spiritual needs of the residents.

MAIN DUTIES AND RESPONSIBILITIES

Case Work Management
Assess and investigate new referrals that may involve home visits
Propose admissions and recommend discharges to the Superintendent prior to such proposal or recommendation being presented at the monthly Residents’ Review Board
Engage in individual case work including giving residents and their significant others a listening ear as needed
Liaise closely with all staff and residents to identify needs and issues, present findings and recommend intervention proposals to Superintendent for preview and approval
Conduct a 6-monthly periodic review of residents’ progress and make recommendations at the monthly Residents’ Review Board
Prepare agenda and minutes of monthly Residents’ Review Board as directed
Provide monthly update of residents’ information for submission to Social Welfare Department
Maintain proper case file records for each resident and ensure that all documentation pertaining to residents are completed in accordance to the requirements of The Salvation Army
Participate as a member of the Residents’ Review Board of the Home and make recommendations on residents’ care plans for Board deliberation and approval

Program Activities Coordination
Liaise with the Superintendent to identify the needs of the residents
Assist in developing relevant programs that would cater to the physical, emotional, spiritual, educational and social needs of our residents
Plan, organize and coordinate activities that meet the needs of the residents
Motivate residents to participate in the activities and encourage them to be involved in The Salvation Army and the Home spiritual ministries
Maintain proper documentation of program and activity schedules
Assist in managing and ensuring the program objectives meet the needs of our residents
Assist in the planning and running of fundraising activities of the Home

Volunteer Activities Coordination
Attend to inquiries of potential volunteers
Recruit, orientate, supervise and liaise with volunteers (individuals/group) supporting our programs
Ensure that volunteer programs align with the different needs of the residents while maintaining the interest of volunteers
Recognize involvement of volunteers on a regular basis i.e. cards/teas/etc.
Maintain proper volunteer applications and database
Residents’ Education Coordination
Responsible for the educational needs of all residents and represents the Home in schools for matters concerning the resident’s education
Work closely with families, and external/internal parties in deriving at an individualized educational plan (IEP) that would best meet and enhance the academic interests and potential of every resident
Liaise with schools and/or Ministry of Education to reinstate residents back to school and provide over-sight in all school referrals and matters of every resident
Document, prepare and present prompt and up-to-date educational inputs of every resident in case conferences, Management and Residents’ Review Board meetings, be it for reporting or discussion purposes
Identify and highlight needs of residents who might require special education and/or extra help with their studies, and coordinate efforts with relevant others, where necessary, in meeting to these needs
Build on the existing in-house resources and develop a system that would allow for residents’ access to materials, computers or resources that would aid in their studies and overall future development
Initiate or undertake any assignments, projects and/or programs that would help promote or enhance the educational element of the Home as a whole, which includes working with volunteers

Other Duties
Assist in the monthly distribution of food parcels to needy families of our residents and families known to the Home as needed
Assist the Corps Officer of Ipoh Corps with their Community Service Outreach Program when requested in consultation with the Superintendent
Represent The Salvation Army in meetings, sponsored events, seminars and other events as and when required
Participate in staff meetings as and when required
Undertake other duties and responsibilities consistent with the position as and when required

The Salvation Army

The Introduction
The Salvation Army made its entrance to Malaya in July, 1938 when Frederick Harvey set up the first centre in a former YMCA building in Penang. He went on to establish a free school on the island for hundreds of poor children, building upon our Founder William Booth’s mission to break cycles of poverty. Over the initial few years the Army opened up corps and social programmes in Ipoh, Melaka, Batang Melaka and Kuching.
Today
The Salvation Army in Malaysia has ten corps and a range of social service programmes that provide both spiritual guidance and practical assistance to people in need. The Army has developed its specialised residential care work with young people and has also developed other services such as homes for the elderly, day care centres, kindergartens and community service centres.
